What are In Home Program Services?

Home care assistance is offered by Yavapai County Community Health Services to help people with chronic disease that are unable to perform their day-to-day activities without assistance.

Personal Care Services

Personal Care Services are available for qualified applicants through the VA Healthcare system, and for self-pay clients. Caregivers hired by Yavapai County provide non-medical, household and personal care services.

  • Bathing and assistance with daily tasks
  • Personal grooming
  • Meal preparation
  • Housekeeping
  • Laundry and changing linen
  • Shopping and Errands
  • Non-Medical Home safety
  • Respite Care

Why is it needed?

When people experience health challenges, staying in their home as long as they can is often the best choice. Our clients, even with limited means, can remain independent in the privacy and comfort of their homes. We customize our level of service to meet the needs of each individual.

Who is eligible?

The VA Medical Provider determines if a veteran qualifies for home care and initiates services. Private pay services are also available.

Is there a cost?

Co-payments may be applicable for veterans served through the Northern Arizona VA Medical Center.

What are the goals of the program?

In Home Program services help support persons in their home. People seek solutions when physical limitations develop from chronic health issues. While nursing homes or assisted living facilities can be appropriate choices, our goal is to offer services that assist a person to remain independent in their home with the kind of support they need for as long as possible.

How are caregivers hired and trained?

A registered nurse who understands home care needs will match clients with trained caregivers. Our staff is thoroughly trained to meet the needs of home-based clients. We require fingerprinting and a background check by the Department of Public Safety.

The caregivers receive ongoing training and interact with the RN supervisor on a regular basis to assist people who wish to remain independent in their homes.

Where are the services for the In Home Programs available?

To qualify, a potential client must live in Yavapai County. Caregivers may be more readily available in the more populated areas such as Prescott, Prescott Valley or Chino Valley, but this program also serves clients in outlying communities such as Black Canyon City or Congress, depending on caregiver availability.
